High Reliability Organization: Make Weak Signals Powerful Enough to Change the Decision

A weak signal needs decision power

High Reliability Organization research treats anomaly reporting, operational awareness, and deference to expertise as parts of the safety architecture

Premise

In high-consequence operations, the absence of a major accident is not proof that the system is healthy. High Reliability Organization research asks how organizations exposed to catastrophic risk sustain unusually reliable performance by noticing small failures, preserving multiple interpretations, remaining connected to current operations, and shifting authority toward relevant expertise when conditions change.

The communication issue is decisive: which anomalies become consequential information, whose observations can travel upward, whether bad news changes resource decisions, and whether rank yields when technical knowledge becomes more important. Reliability is therefore organized through information flow and authority as well as through equipment, procedures, and individual competence.

Model

In this series, Model names the editorial family primarily used to understand and explain communication: theories, frameworks, effects, traditions, principles and other conceptual resources. It is an umbrella for a route into the field, not a claim that every included object is a predictive model.

Analytical Frameworks, Typologies & Heuristics

Organizes dimensions, distinctions, types or questions so that a situation can be examined systematically. A framework supplies structure, a typology distinguishes forms, and a heuristic offers a usable way to orient judgment.
